This survey is completely anonymous - you do not have to write your name on it and the information you provide will be confidential. However, if you tell us about a serious crime that has not been reported to the police, we may be required by law to tell the police about it. Some of the questions just need a tick to answer them, while some others will direct you to go on and answer another question. Sometimes we would like more information to help us understand, so we have left a blank box for you to write in. All the information you can give us will help.
